Q: Can you get plastic surgery to remove dark eye bags on your face in New Jersey?

A: The appearance of darkness under the eyes is usually caused by the shadow under the eye bags which are due to excess or herniated fat. This can be corrected by lower eyelid surgery where some of the excess fat is removed or used to fill in the groove under the eyelid. At the same time the skin of the lower lid can be tightened. The fillers like Restylane could be used in milder cases where there is only a groove but not excess fat. This area is very prone to deformities after injecting fillers. One needs to be extremely conservative and careful because I have seen many patients coming with problems after this type of injection was done by doctors who are not aware of the pitfalls in this area. I use a blunt cannula to minimize pain and bruising. This technique is also much safer as it prevents inadvertent intra-vascular injections.

Q: Would you have Plastic Surgery done to improve the appearance of your face (make you look younger or just different)?

Plastic surgery procedures such as eye lift, brow lift, facelift, cheek implants, chin implants, Botox injection, mole removal, skin resurfacing Nose restructured….

A: The answer is yes. Patients often ask me would you do this on your wife your sister and my answer is that I would never propose to do a procedure on a patient that I wouldn’t feel comfortable to offer to my closest relatives or to myself.

Q: What is better in the nasolabilal folds Radiesse or Restylane?

What is the difference between having Radiesse vs Restylane filler for nasolabial folds? Which is better? I’ve had it done with Juvederm but have read that the other two are longer lasting.

A: Radiesse is better in this area because it lasts longer. Juvederm ultra plus is longer lasting than Restylane and would be my second choice.

Q: Bruising with Radiesse. Is bruising normal after injection of Radiesse.

I have a huge bruise on my chin from radiesse injections. Is this normal? I also wondered if it’s the injector’s fault that caused the bruising.

A: Bruising is unpredictable but it can happen. It is not the surgeon’s fault. It rarely happens to the same extent on both sides. If you take aspirin within the week before injection the bruising is more likely. I use ice before injection. It seems to help with the pain and bruising. I also use a blunt cannula which makes it painless and there is almost no bruising.

Q: Radiesse for Under Eye Bags?

I’m considering getting radiesse under my eyes to help with the Bags…..what is your opinion. I’ve been reading a lot of reviews that don’t suggest it under the eyes. I’m 27 so my skin should not be too thin. I’m considering getting Radiesse under my eyes to help with the Bags.

A: Radiesse is unforgiving around the eyes. It can be used successfully deep on the bone to fill in grooves but the injector has to be very experienced. I would favor Restylane. But you have to understand that all fillers could be problematic around the eyes and you have to choose your doctor very carefully. I do these injections with a blunt cannula. This technique makes the procedure safer, less painful and produces less bruising.

Q: Radiesse for Nasolabial Folds. Is Radiesse good for nasolabilal folds.

I am thinking about Radiesse for my nasolabial folds and possibly marionette lines. Does Radiesse work for these areas?

A: Radiesse is the ideal filler for the nasolabial folds and lasts the longest. It needs to be place deeper than the HA filers.

Q: How Long Does Radiesse Last?

I have had Radiesse put under my eyes and came up with redness and bumps. Will this REALLY go away. I hate to look at myself and explain it. I’m very depressed over this!! It has been 2 an half mo. I’m hoping I don’t have to wait a year or more. I can’t stand what happened and is their an answer out their??

A: Radiesse is a great filler but it requires more skill and knowledge than other fillers. It can be used around the eyes but it has to be placed carefully in small quantities and deep on the bone if not it will leave swelling and palpable lumps. If your lumps are visible they can be removed through a small incision. All fillers last longer around the eyes. I do not use this product under the eyes, but I do use it in the upper cheeks where there is more soft tissue cover.

Q: Radiesse Vs Juvederm for Lips?

Which do you think is better for lips, Radiesse vs Juvederm?

A The best filler for lips.

Radiesse is a great filler in the face that I use a lot but I would not use it in the lips. It can form lumps that could be felt but sometimes seen. My favorite filler for the lips is Restylane which allows me to shape the lips better than with Juvederm.

Q: Is It Possible to Reduce the Ear Lobe Size?

If ear lobes are too large, can they be cut away to reduce size while also pinning back and/or reshaping the whole ear?

A: This is a simple operation that can be done under local anesthesia with very little discomfort. Most of the time this procedure is performed at the time of a face lift as the ear lobes tend to get larger and more redundant with age.

Q: Anesthesia Used During a Facelift?

My doctor said he could perform my facelift under local anesthetic and just give me some sedation. I am 42, would general anesthesia be safe for me? I don’t know what is worse – the general anesthesia, or not being fully knocked out.

A: Can a facelift be done with local anesthesia.

The answer is a big yes. I perform 80% of my face lift procedures under local anesthesia with a sedation that is administered by a nurse following my orders. I work in a fully certified operating room and use a monitoring nurse who is ACLS certified. The patients are awake and can cooperate. They are relaxed and usually describe the time in the operating room as interesting. There are a few special techniques in giving the local anesthetic that make the injections more tolerable. When the local anesthesia is given properly the operation is painless and easy. There are less incidences of nausea and vomiting with this method.

Q: 1-Step Facelift vs Full Facelift

I was considering the lifestyle lift, but read too many negative answers. Dr. XYZ has a 1-step alternative also. should I ignore these 1-step procedures and go w/ a full procedure ? money is a problem, though.

A: What type of facelift is appropriate.

There are as many facelifts as there are patients. What I mean is, I hardly ever do the same face lilt on 2 patients. Everybody needs a slightly different surgery. So it’s a matter of finding a plastic surgeon whom you can trust and let him do what’s best for you. Generally one has to look where the problems are and correct them one by one. The more problems areas there are the more extensive the operation needs to be. Don’t trust the facelifts with a name.

Q: Is There a Non Surgical Face Lift That Works?

I am in my early 50s with drooping facial features. I pray that someone tells me that a non surgery face lift is available, an alternative that doesn’t require going through lots of recovering. May be I can get away with treating using filling with things like Restylane?

A: Non surgical facelift. Does it work?

When the human face ages, several things happen. We loose fat, muscle and bone mass, but we also loose skin elasticity. It really depends on the degree of aging. The fillers can help and give improvements by helping restore volume, but often one needs to correct the skin relaxation and lack of recoil. This is why you need to go to a plastic surgeon to get a complete analysis of your condition so that he can offer you the best solution. There is no magic wand that can restore youth without any recovery.

When a mid face lift is done effectively, it has to last forever because if not, the effects can be disastrous. The reason is that the mid face lift is often done with a lower blepharoplasty so that the excess laxity under the lower lid can be removed. If the results of the mid facelift did not last, then the lower lid would be pulled and this would cause an ectopion or at least a deforming scleral show. Many different techniques exist but you have to make sure you go to the surgeon with the best experience who is confident that his result will last.

How long after I stop using Retin-A can I tan in the sun or tanning bed?

A: Tanning after Retin A

If you are using Retin A properly you should have been instructed not to be exposed to excess sun. The use of Retin A is typically done over a period of time, this results in thinning of the protective keratin layer as well as reduction of the pigment cells. Both these facts will leave you more vulnerable to the UV radiation. Not only you should not tan, but you should protect your skin with a complete sunblock to avoid damaging your skin permanently.

If you stop using Retin A and your skin regains its normal color and texture then you can tolerate the tanning better. However, this is not a good thing to do for your skin in the longrun.
